These Terms and Conditions govern your use of this website and the online booking of appointments with Dr. Coby Sheily DDS, 4852 Melrose Avenue, Los Angeles, California 90029. By using this website or booking an appointment, you agree to these terms.

Not medical advice

Content on this website is for general information only. It is not a diagnosis, treatment plan, or substitute for an in-person examination. All treatment is subject to examination, diagnosis, clinical appropriateness, and availability. Results vary from person to person and are not guaranteed.

Emergencies

This website is not for medical emergencies. If you have difficulty breathing or swallowing, severe swelling, uncontrolled bleeding, or any life-threatening condition, call 911 or go to the nearest emergency room.

Appointments and deposits

• The practice is open Friday through Sunday, 10:00 AM to 4:00 PM Pacific Time. Online bookings are requests until confirmed.
• A $150 deposit is required to reserve an appointment. The deposit is credited in full toward your treatment.
• You must provide accurate, current information when booking. Appointments for minors must be made by a parent or legal guardian.

Cancellations and missed appointments

Appointments may be cancelled or rescheduled at least 48 hours in advance. Late cancellations and missed appointments may result in forfeiture of the deposit. Full details are in our Cancellation Policy.

Fees and payment

Treatment fees are discussed with you before treatment begins. You are responsible for payment of all fees at the time of service. Payments are processed by a third-party payment processor subject to its own terms.

Use of this website

You agree not to misuse this website, including attempting to gain unauthorized access, interfering with its operation, submitting false bookings, or using it for any unlawful purpose.

Intellectual property

All text, images, logos, and design on this website belong to Dr. Coby Sheily DDS or its licensors and may not be copied or reused without written permission.

Disclaimer and limitation of liability

This website is provided "as is" without warranties of any kind. To the fullest extent permitted by law, the practice is not liable for any indirect, incidental, or consequential damages arising from your use of the website. Nothing in these terms limits liability that cannot be limited under California law, including professional responsibility for the care you receive.

Your use of this website is also governed by our Privacy Policy.

Governing law

These terms are governed by the laws of the State of California. Any dispute will be resolved in the courts located in Los Angeles County, California.

We may update these terms from time to time. Continued use of the website after changes means you accept the updated terms.
